Pursuant to RSA 236:l3 and regulations adopted hereunder, I apply for permission to construct a driveway entrance as described in this permit. I understand the conditions of this application and as printed on Page 4 of 6 of this Application.

As the landowner, I hereby agree to the following conditions:
1. To construct driveway entrance only for the bona fide purpose of securing access to private property such that the highway right-of-way is used for no purpose other than residential travel.

2. To construct the driveway entrance at permitted location in accordance with all Town statutes, all provisions of Driveway Permit of Driveway Permit specifications and standard drawings for driveway entrances issued by the Raymond Public Works Department.

3. To hold harmless the Town of Raymond and its' duly appointed agents and employees against any action for personal injury and/or property damage sustained by reason of the exercise of the Town Driveway Permit.

4. To furnish and install drainage structures that are necessary to maintain existing highway drainage and adequately handle increased runoff resulting from development and so that no drainage runs onto the Town roadway.

5. To leave the highway right-of-way of the Town of Raymond free from all debris such as stones, earth and brush resulting from the construction of such driveway.

6. The owner agrees to maintain the driveway culvert in good and operating conditions, regardless of the culvert location.

7. Driveway Permits are non-transferable and are valid for only one year from date of approval.

8. Property owner's agent must submit written proof that he or she is an authorized agent.

Fill out the attached Driveway Permit Sketch and submit the entire form along with a check in the amount of $30.00 and payable to the Town of Raymond, to: Public Works Department, Town Office, 4 Epping Street, Raymond, N.H.

Driveway Permit Application Instruction Sheet

1. Print or type application (Page l)

2. Flag driveway centerline (for recognition by the Public Works Department - stakes with surveying ribbons, paint on the roadway, or similar techniques are acceptable.

3. Complete the information required on the map (Page 2)

a. Label North (N) on the compass.
b. Label the distances from centerline of the proposed driveway to both property lines.
c. Label distance to the nearest utility pole; read the letters and numbers on the face of the pole and record as shown.
d. Label the road name.
e. Label the approximate sight distance.
f. Label the width of the proposed driveway.
g. List type of driveway surface.
h. Label, by use of an arrow ( ), the direction of drainage in the roadside ditch.
i. List any other information which may be of help to the Public Works Department.

4. Review the sample driveway sketches and typical driveway details.

5. Submit application and attached map (Pages l and 2) plus Pages 3 to 6 to the Public Works Department for approval along with a check in the amount of $60.00 made payable to the Town of Raymond.

Once the Driveway Permit Application has been approved by the Highway Supervisor, a Driveway Permit will be issued and:

1. The applicant may construct the driveway as required.

2. Once the driveway has been constructed, phone the Public Works Department to arrange for an inspection and approval of the constructed driveway. (NOTE: a Certificate of Occupancy will not be issued without this final approval by the Public Works Department.

3. Applicant's driveways which are not completed or acceptably constructed at the time of Certificate of Occupancy issuance will be required to provide a driveway bond for outstanding or unacceptable work.

CONDITIONS FOR CONSTRUCTION



1. All drives shall be constructed to conform to typical driveway sections and details for the Town of Raymond, N.H. (see pages 5 and 6 attached).

2. A minimum sight distance of two hundred ( 200') feet shall be required for all proposed and newly installed driveways.

3. Only one driveway entrance per lot is permissible. There shall be only one point of access/egress per lot.

4. Driveways shall be fourteen foot (l4) ' in width. Driveway entrances shall flare as it approaches the pavement to a maximum width of twenty feet (20') measured at the existing edge of roadway pavement.

5. If wetlands are crossed or infringed upon, a dredge and fill permit shall be required from the State Wetlands Board.

6. Where required, driveway culverts shall be a minimum of twelve inches (12") in diameter and twenty feet (20') long; and must be constructed of either corrugated metal or reinforced concrete.   Culverts made of aluminum or plastic shall not be acceptable.

7. Headwalls constructed of stone rubble and mortar, conforming to State of New Hampshire, Department of Transportation, Standard Specifications and Details for Construction of Bridges and Highways shall be installed at both ends of all culverts. All driveway headwalls shall be constructed to the same elevation as the driveway. The width shall be determined by the depth of the culvert.

8. The driveway shall slope away from the edge of the paved roadway at a slope of 2% for a distance of twenty five (25') feet, and be paved to the depth of two (2") inches from the existing road pavement edge to the applicant's property line.

9. Driveways not completed at the time of Certificate of Occupancy issuance shall require the owner to provide the Town of Raymond with a cash bond to cover unacceptable or incomplete driveway construction prior to certificate release. Said cash bond shall be based at a minimum on the following stipulated values:

 Pavement completion  $l,000.00
 Culvert completion  $l,000.00
 Head wall completion  $l,000.00
 Excavation & filling  $ 500.00
 Loam, seed, establish growth  $ 500.00


10. All driveways shall intersect the Town's roadway at a perpendicular angle to the center line of each.
